Seizure First Aid Competition – 2010 Australian Corporate Games
1. The competition is a Trade Promotion competition of the Epilepsy Foundation of Victoria and instructions on how to enter and other details contained within promotional material and on the Epilepsy Foundation of Victoria website www.epinet.org.au for the competition form part of the conditions of entry.
2. Entrants must be participants or members of a team participating in the 2010 Australian Corporate Games and by entering the competition, entrants agree to abide by these terms and conditions. Employees of the Epilepsy Foundation of Victoria and their respective spouses or dependant children are ineligible to enter.
3. The prize is a JB Hi-Fi shopping spree in the form of gift cards to a maximum value of $650.00.The prize will not be exchanged for cash.
4. To claim the shopping spree the winner may have a choice of JB Hi-Fi retail outlet provided they are able to accept JB Hi-Fi gift cards. If the winner is a person under the age of 18 years, the prize will be awarded to a parent or guardian of the winner.
5. Substitute prizes will not be available, should the winner be unable to use the prize at the allocated time.
6. In the event that the winner is not based in Melbourne, the recipient of the prize may be required to travel to Melbourne at the winner’s cost to collect or redeem the prize.
7. The Epilepsy Foundation of Victoria may at its absolute discretion substitute a different prize in place of any prize referred to in these terms and conditions.
8. The competition commences 8.00am (EDST) Thursday 18 November 2010. Entries for this competition close 5.00pm (EDST) Friday 26 November 2010. No late entries will be eligible.
9. Entrants will be eligible to win the prize if the Entrants
(a) are participants in the Australian Corporate Games and by depositing their business card or by completing the coupon provided in the Epilepsy Foundation of Victoria barrel during the Corporate Games registration process; or
(b) are participants in the Australian Corporate Games and by completing the online registration form provided on the Epilepsy Foundation of Victoria website, www.epinet.org.au; or
(c) are participants in the Australian Corporate Games and by completing the official registration coupon and faxing it to 03 9882 7150 or by mailing it to the Epilepsy Foundation of Victoria 818 Burke Road Camberwell 3124.
10. The draw will be conducted at 2.00pm EDST on Tuesday 30 November 2010 at EFV. The judge’s decision will at all times be final and no correspondence will be entered into.
11. The winner will be notified by telephone and will receive confirmation by mail or email. The winner will also be published on the www.epinet.org.au website from 5pm on the date the winner is drawn and in a newspaper circulating generally in Victoria on Thursday 2nd December 2010.
12. It is the responsibility of each winner to collect the prize. If unclaimed, the prize for this competition will be automatically forfeited after 180 days from the date of notification and will not be available if claimed after that date.
13. It is a condition of entry that Epilepsy Foundation of Victoria has the right to publicise the winner for any promotion or matter incidental to the competition, if required. The winner consents to their name being put on the www.epinet.org.au website.
14.The winner may also be required by Epilepsy Foundation of Victoria to participate in a photo session and acknowledges that Epilepsy Foundation of Victoria has the right to use such publicity photos in any medium (including, without limitation, the Internet), and in any reasonable matter it deems fit. The winner agrees to make themselves available for publicity purposes without charge.
15. All entrants release and indemnify Epilepsy Foundation of Victoria and all of its related companies and personnel from and against, all liability, cost, loss or expense arising out of acceptance of any prize(s) or participation in the competition including (but not limited to) loss of income, personal injury, damage to property and whether direct or consequential, foreseeable, due to some negligent act or omission or otherwise.
16. Neither Epilepsy Foundation of Victoria nor any of its related companies or personnel will be responsible or liable if for any reason beyond their reasonable control any element of any prize is not provided.
17. Neither Epilepsy Foundation of Victoria nor any of its related companies or personnel will be liable to any person for any loss or damage sustained or incurred if, for any reason, the competition is suspended, varied or terminated at any time, with or without prior warning or publicity.
18. Neither Epilepsy Foundation of Victoria nor any of its related companies or personnel will be liable to any person for any loss or damage sustained or incurred arising from late, lost or misdirected entries.
19. Neither Epilepsy Foundation of Victoria nor any related companies or personnel will be liable to any person for any loss or damage sustained or incurred arising from any change in the value of a prize occurring between the publishing date and the date the prize is claimed.
20. Epilepsy Foundation of Victoria reserves the right to exclude any person from this competition for any reason whatsoever.
21. In all matters, all decisions of Epilepsy Foundation of Victoria will be final and cannot be contested. No correspondence or discussions will be entertained.
22. Epilepsy Foundation of Victoria reserves the right to amend these terms and conditions or cancel the competition at any time after this competition has commenced.
23. If for any reason this competition is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorised intervention, fraud, technical failures, or any other causes beyond the control of Epilepsy Foundation of Victoria which corrupt or affect the administration, security, fairness, integrity or proper conduct of this competition, Epilepsy Foundation of Victoria reserves the right in its sole discretion to disqualify any person who tampers with the entry process. Epilepsy Foundation of Victoria assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorised access to or alteration of entries.
Privacy statement
At Epilepsy Foundation of Victoria, we respect the privacy of your personal information and are bound by the National Privacy Principles (NPPs) set out in the Privacy Act 1988.
On entry for the Epilepsy Foundation of Victoria Australian Corporate Games 1st Aid promotion, we will collect details of your name, address, age, telephone number, and in some cases other contact details such as a facsimile number, post office box number and/or email address.
Epilepsy Foundation of Victoria will use your information to send you Epilepsy Foundation of Victoria information on first aid for seizures. We may use the information to send you further information about our work including fundraising materials. We may use the information to publish the results of the competition in the local newspapers or on the internet. If at any time you do not wish to have further materials or information sent to you then your details can be removed by request in writing to 818 Burke Road Camberwell 3124 or by email epilepsy@epilepsy.asn.au.
If you do not wish to provide personal information which is required, Epilepsy Foundation of Victoria may not be able to properly register you for the draw, or contact you in case of changes to the promotion. You have a right to access your personal information which is held by Epilepsy Foundation of Victoria.
The personal information collected by Epilepsy Foundation of Victoria in connection with this competition, and this statement in connection with the information, is separate from, and without prejudice to, any other personal information concerning you collected and held by Epilepsy Foundation of Victoria.
