Epilepsy: know me, support me

For Disability Services and Families

The Epilepsy Foundation in partnership with Bendigo Community Health Services has developed the Epilepsy: Know me, support me suite of resources targeted to the Australian disability sector, as part of the National Disability Insurance Scheme (NDIS) practical design funding round.

These resources aim to provide both people with a disability living with epilepsy, their supporters and disability services,  tools that enable the person to better understand their epilepsy and have increased involvement in saying how they want to be supported.

 

Developing an Epilepsy Management Plan: a resource for support workers and families

This interactive booklet will help the support person engage the person with a disability in the development of the epilepsy management plan.

Epilepsy Management Plan (EM) Template

There are five EMP templates available, each having space for between 1 and 5 different seizure types.  You need to decide how many different seizure types the person has first and then download the relevant one.

Step 1:  Identify how many different seizure types the person has

Step 2:  Select the EMP that corresponds to the number of seizure types

Step 3:  Download the appropriate EMP and guidelines

Step 4:  Complete the EMP electronically and 'save as' on your computer for future reference/reviews

Step 5:  Print and take to your doctor for endorsement

Epilepsy Wellbeing Map

This resource helps the support person to prepare for an epilepsy appointment and comprehensively outlines all the key aspects of a person’s health and wellbeing.  It is available online or in hardcopy and has recently been produced by UCB Pharma.

Epilepsy Management in the Disability Sector: a resource for better practice

Individuals and supporters can use the better practice resource to assist them in their decision when choosing a service provider.

Service providers can benchmark themselves against the better practice resource and identify ways to enhance the person's support and epilepsy management.

For the Person Living with Epilepsy

The resources you will find on this site have been developed following input from many people living with epilepsy through an Australian-wide survey to find out what epilepsy supports people living with epilepsy and their families wanted from disability services.  We also surveyed disability services to gauge their understanding of epilepsy and capacity to support people with epilepsy.

This booklet describes epilepsy in straight forward language with the use of pictures.
